I printed this out on colored paper to keep in their folders and we use this when I read a story to identify and explain theme.

Teaching about theme - some great ideas here.  I don't like the one word "themes" on the one anchor chart since they don't correspond to the definition of theme ("the message, moral or lesson..."), but there are some excellent ideas for teaching theme and assessing it here.
